END USER LICENSE AGREEMENT AND DISCLAIMER

IMPORTANT: PLEASE READ THIS END USER LICENSE AGREEMENT AND DISCLAIMER ("EULA") CAREFULLY BEFORE USING THE TRACKHEALTHAI APPLICATION ("APPLICATION"). BY USING THE APPLICATION, YOU AGREE TO BE BOUND BY THE TERMS OF THIS EULA. IF YOU DO NOT AGREE TO THE TERMS OF THIS EULA, DO NOT USE THE APPLICATION.

1. LICENSE GRANT

Subject to the terms and conditions of this EULA, TrackHealthAI LLC ("Company") hereby grants you a limited, non-exclusive, non-transferable license to use the Application solely for your personal, non-commercial use.

2. RESTRICTIONS

You may not:

Copy, modify, distribute, sell, or lease any part of the Application
Reverse engineer, decompile, or disassemble the Application
Remove or alter any proprietary notices or labels on the Application
Use the Application for any commercial purpose without written consent

3. MEDICAL DISCLAIMER

The App is not a medical device and does not provide medical advice, diagnosis, or treatment.
Information provided by the app (including appointment scheduling, reminders, or health episode tracking) is for informational and organizational purposes only.
The App does not replace consultation with licensed healthcare professionals. Always seek the advice of your physician or other qualified healthcare provider with questions regarding a medical condition.
Never disregard professional medical advice or delay seeking it because of information presented in this App.
